Undereye filler is a contentious treatment!

Truthfully- most people who are bothered by under eye concerns won’t get a great results from dermal filler. It is the single most important area for rigorous assessment and consultation prior to treatment, as each person has a different underlying medical history and anatomical considerations- not all patients are suitable candidates. And in time, it is possible that the great result you had initially will become a puffy- this is due to the anatomy of the region. In this case, the filler can be removed.

However, it’s best to avoid this situation by only treating good candidates, supporting the area first with midface and cheek support (with dermal filler) and only treating the under eye region with a small amounts.

It's crucial to have an honest discussion before undergoing under eye treatments. Certain factors can make a person a poor candidate for the procedure, such as a

🤧 history of upper airway allergies or obstruction : If my patients have allergy, I will often trial an intranasal steroid and decongestant first to see if this helps. I have tried nasal “tox” on some patients with limited success. If there is a clear source of allergen e.g. dust, cats, pollen, a referral to an allergist may be helpful. If there is significant obstruction, a referral to an ENT to discuss surgical options is required. Upper airway causes of dark circles and tired eyes are SO COMMON but not often discussed. This is one benefit from seeing an aesthetic doctor who has worked as a GP or in allergy clinics (I have done both and the benefit of this experience comes in handy.

Tixel for thin under eye skin is a good option for improving skin quality

🛍️ Poor skin quality (thin, papery skin) Eye bags, or significant laxity of the skin or muscle, or the presence of eye bags require alternative treatments or referral to a surgical specialist.

Tixel will help to improve crepey thin skin and I do advise patients so this prior to fillers under the eyes if the they have poor skin quality or skin laxity on the upper or lower eyelid skin.

It's important also important to note that dermal fillers won’t reduce pigmentation or blood vessels (vasculature) in the skin.

While fillers can alter light reflection, and reduce the look of pigment, complete elimination of dark (melanin or haemosiderin) or blue/red (vascular) pigmentation is not achievable. Lasers and skin care may help with these concerns, but lasers in this area is dangerous and skin care has limited capacity to help.

EyeMax AlphaRet Overnight Cream is probably the best night cream I have used to improve skin quality and reduce superficial skin pigmentation. Redness or vessels around the eyes may be related to an inflammatory response to makeup, diet or your environment. Deep pigment won’t improve with topical skin care- this is from metabolic issues.

If we choose to perform dermal fillers, it’s generally done in 2 stages- supporting the area first with midface and cheek filler, and then a small amount of undereye filler which is usually done a few weeks later. It's also important to avoid overcorrection, as this can result in puffiness and an unnatural appearance. Treatment with fillers in the area is certainly a case of #progressnotperfection, aiming for less is more and being happy with small changes.

In this specific case, this young patient has a volume deficit in the under eye area, which made her look tired. After a thorough medical and facial assessment, a small amount of dermal filler was used to address the volume loss. We had a frank discussion that this would not improve pigmentation. We were both happy with the outcome :)
